It contains two forms of data collection and storage, which one book of life and theatre book. In the book of life, the data collection is collected during birth registration while the death information to be collected after death. The usefulness of the theatre book is for birth; it contains information such as name of hospital, member, date, age, sex e.t.c. Diagnosis operation amongst had given by surgeon assisted by its assistance. The theatre book is a record that contains the birth registration of a new registration of a newborn baby.
